Hourly Pay Range: $18.00 - $20.00

As directed by the RN, the Nurse Associate will assist with the monitoring and coordination of the delivery of health care services for individuals with developmental disabilities who are living in community-based homes. The Nurse Associate will accompany the site RN to weekly site visits and will work in coordination with the site RN to complete tasks associated with the RN responsibilities. The Nurse Associate will be responsible for working with the site Medical Liaison/Team Leader as directed by the site RN.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES:

  • Assists the RN with coordinating the delivery of preventive and routine health care for individuals with developmental disabilities.
  • Completes RN directives within their scope of practice. Ensures that medical follow up as indicated on PNO’s/Verbal Orders/etc. is completed at the direction of the RN.
  • Follows up on documents in Medical Communication Book, SharePoint Folders, and EHR.
  • Follows up on lab and diagnostic testing, review and track results.
  • Completes nursing care documentation under the direct supervision of the site RN
  • Assists with coordinating medical program needs with the sites ML/TL, site management, community services, medical providers and other related agencies as part of the directives from the assigned RN.
  • Assists with ensuring adequate scheduling and completion of health-related exams, appointments, laboratory/diagnostic testing as ordered or required.
  • Communicates issues and carries out all directives of site RN and Nursing Administration.
  • Participates as an active member of the Interdisciplinary Team.
  • Maintains compliance with and assists with ensuring facility compliance with all applicable State/Federal regulations and agency policy and procedure.
  • Maintains communication with Medical Liaison/Team Leader and site RN
  • Participates in various nursing workgroups as assigned.
  • Completion of AMAP certification.
  • Performs CPR.
  • Complies with all agency policies and procedures.
  • Other duties as required by Nursing Administration.

MINIMUM QUALIFICATIONS:

  • High School Diploma
  • Will need current CPR certification or must attain CPR certification within one month of employment.
  • Minimum of one year of experience working in a position in a healthcare or human service related field.
  • Driver’s License that meets agency policy.
  • Ability to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling and climbing.
